Scholarship Reinstatement Form
This form is designed for individuals who were recipients of the MTES scholarship at some point in time, but are currently ineligible to receive funds due to being on a scholarship deferment or having become academically ineligible. This does not include students who have graduated with a degree in Education or became ineligible for non-attendance to the required annual symposium. Along with completing this form, individuals requesting reinstatement must provide support documentation for the request, which is usually in the form of a copy of your registration. This documentation should be emailed to cwilliams@ffmt.org. if additional documentation is needed, you will be contacted directly via email.
